Sam Grey is the product of a one night stand between teenagers Kelsey Jefferson and Bobby Warner. A pregnant Kelsey came to Pine Valley, where Anita Santos introduced her to Edmund and Maria Grey. Unable to conceive, Edmund and Maria planned on adopting Sam. Kelsey agreed to the plan, but some months after Sam was born she exercised her legal right and reclaimed her son. Bobby managed to convince Kelsey to sign full custody over to Edmund and Maria, who adopted Sam.
Sam was devastated when Maria was presumed dead after a plane crash. In reality, she had survived but lost her memory. After five years, Maria resurfaced in Pine Valley and eventually regained her memory. Sam left for an exchange program in France, and during his absence his parents' marriage crumbled. Edmund was murdered, supposedly by Maria's ex-lover Zach Slater, and Sam returned home for the funeral. Sam was furious with Maria and blamed her long ago romance with Zach for providing the motive for Edmund's murder. Eventually Zach was cleared of the crime, but Sam's anger toward his mother continued. Sam demanded that Maria tell him who his adopted parents were, and Sam was shocked to learn that his uncle Bobby was his father, while he was a Martin on his mother's side. Sam sought to live with his Great Uncle Tad, who refused and ordered Sam to work things out with Maria.
Sam met Lily Montgomery shortly after his return home, and became enchanted by her. Lily was autistic, and Sam grew to appreciate her unique way of looking at the world. Sam ran away from home, and Lily hid him out. Before being found by Aidan Devane, Sam tried to kiss Lily and let his attraction be known. Lily, who normally disliked touch, agreed to Sam's kiss. Lily enjoyed the kiss and asked Sam to be her boyfriend. Sam agreed.
Maria decided to move to California, and agreed to let Sam stay in Pine Valley and get to know the Martin family. Sam and Lily continued to grow close. Sam was upset when Lily thought she saw a ghost at Wildwind, and assumed it was Sam's father. Still reeling off the pain of his dad's death, Sam yelled at Lily. Lily was upset, but Sam eventually apologized and the two made up. Sam soon learned that Lily actually did see something at Wildwind---his Aunt, Julia Santos Keefer. Julia was on the run from the Witness Protection Program, determined to find her husband's killer. Sam pledged to help her in any way he could. He was furious when he learned Zach was helping Julia.
