This is an interview report paper FORMAT submitted to one of my reqts in PATIENT-DOCTOR MODULE I (a med subject): A PATIENT PROFILE FOR PSYCHIATRY DEPTA.CHIEF COMPLAINT: The patient (myself) is suffering from severe addiction to friendster with a duration of 2 months not to mention his Attention Deficit Hyperactive Disorder ( ADHD ) and of being a certified smart crazy asshole jackass medical stud.B.SOURCE AND RELIABILITY: The data came from the patient himself with a bad reliability.C. PATIENT PROFILE: In addition with the above mentioned data, the patient's diet mainly includes protein shakes (whey pro powder, banana and boiled eggwhite), red meat (no pork ) carbo and less fat (hahaha). He sleeps around 4-5am (insomniac) and wakes up around noontime. His everyday activity is roaming around in differnt places specifically in mall, bars, theaters, record bars, gimik places, etc. He used to workout but stopped due to his hectic sched in medical school, he stopped almost half a year . He loves to smoke a lot but refrains or minimizes in drinking beers lately (ubo! ubo!). Am a certified jackass party clubber boy and a gym rat! .... ahhahaha mga gaguuUuUuu!!!
